What's Included

  • 60-minute comprehensive preventive physical

  • Full review of your medical, surgical, family, and social history

  • Age-appropriate preventive screening orders (mammogram, colon cancer screening, bone density, and others as appropriate)

  • Labs included: CBC, CMP, HgbA1c, lipid panel, TSH & Free T4, vitamin D, vitamin B12 (to be completed within 30 days)

  • Full lab result review

  • A personalized written Health Optimization Plan — nutrition, activity, sleep, stress, and evidence-based integrative recommendations

Optional add-on: Pap smear — $80

Membership may be a better fit if you:

  • Manage a chronic condition (high blood pressure, diabetes, thyroid disease, asthma, autoimmune disease, etc.)

  • Take regular prescription medications

  • Want same- or next-day appointments and direct access to your doctor year-round

Is The Long Physical Right for You?

Good to Know

The Long Physical is a preventive wellness visit, not a substitute for ongoing primary care — it doesn't include chronic condition management, follow-up care, medication management/refills, or urgent/sick visits. If something is found during your visit that needs ongoing care, we'll walk you through next steps, and enrollment in the DPC membership is required to continue treatment (a short-term prescription bridge, up to 30 days, may be provided in the meantime when appropriate).

Join membership within 30 days

Your $299 visit fee is credited toward your first month if you join a Primary Care membership within 30 days. After that, standard enrollment applies (one-time $69 fee).
